Once we have scheduled your initial appointment, I will mail you the initial paperwork or you can choose to come early and complete the forms at my office. If you would prefer, you can download them by clicking the links below.
Individual Therapy: please complete the Patient Information Form and review and sign the Consent to Treatment.
Couples Therapy: each partner needs to complete the Patient Information Form and review and sign the Consent to Treatment.
Teen and Family Therapy: the parent or teen needs to complete the Patient Information Form and review and sign the Consent to Treatment.
If information from other professionals is required, please complete and sign the Authorization for Use and/or Disclosure of Information.
